Primary Care Billing

Co-pays, co-insurance and any patient responsibility balance are due at the time of service. For your convenience, we accept cash, debit card, Discover, Visa or MasterCard.

You will receive a paper statement each month for any balance due on your account. This includes amounts for services billed to your insurance which have not been paid after 90 days from the date of service.

Motor vehicle and third party injury claims are your financial responsibility. We will provide documentation for you to submit as evidence of services rendered. We do not do third party billing.

  • Patient account number
  • A brief summary regarding any information you wish to provide us, or the nature of your question or concern

A representative from EvergreenHealth Professional Billing will follow-up with you as appropriate within two business days.


Special Payment Arrangements

We understand there are times you may not be able to pay your bill in full.

In those instances, it is your responsibility to call our Business Office to make special payment arrangements.

Delinquent accounts are turned over to a collection agency following non-payment of a second statement.


Questions about Bills from other Providers

While you may have been treated at EvergreenHealth locations, some imaging, lab test, procedures or other services may have been performed by other groups that regularly provide care for EvergreenHealth patients. 

For example, many independent physicians, ambulance companies and labs are separate business with their own billing and account procedures.

Get a list of the groups that regularly provide care for EvergreenHealth patients.  If you receive a bill from them and have questions about it, please contact them directly.
