EvergreenHealth provides an employer-paid basic life plan that insures employees for 1 times their annual base salary.
AD&D coverage pays an additional 1 times the annual base salary to employees should they suffer loss of life or certain injuries as a result of an accident.
Employees may purchase additional life insurance in multiples of salary (1x, 2x, 3x, etc.) up to a maximum of $350,000 or 5 times annual salary, whichever is less. Dependent coverage is also available.
Rates are determined based on the age of the employee/dependent and coverage level.
EvergreenHealth offers a voluntary short-term disability option to all benefit-eligible employees.
The plan provides up to 60% salary replacement after a 30 days waiting period if you become disabled and unable to work.

EvergreenHealth provides employer-paid long-term disability coverage to all benefit-eligible employees.
Long-term disability replaces 60% of your salary up to a maximum of $15,000 a month when you're disabled and unable to work for an extended period.
LTD replaces 40% of your salary for non-supervisory positions with an option to buy-up to 60%.
As a public hospital district, EvergreenHealth sponsors a 457 Deferred Compensation Plan available to all full-time, part-time and per diem employees.
Employees may defer up to $17,500 or 50% of their annual compensation, whichever is less on a pre-tax basis.
Employees 50 or older may defer an additional $5,000.
This plan is available to all full-time and part-time employees scheduled to work at least 20 hours a week, upon completion of one year of continuous employment.
Enrollment is the first of the quarter upon meeting the eligibility requirements.
EvergreenHealth matches 50% of employee's pre-tax deferrals up to a total contribution of 4%; for supervisors and above, the match is 100% up to 6%.
