A visit with a pharmacist provider can help improve your care.

Pharmacist providers support patients with chronic conditions such as diabetes, high blood pressure and high cholesterol manage and monitor their medication therapies.

Their highly specialized knowledge of medications can help in managing medication therapies, monitoring medication results, adjusting dosages and preventing dangerous drug interactions. This may help you reduce your health care costs.

Our pharmacists are board certified/board eligible in Pharmacy Specialties with years of college and post-collegiate training as a registered pharmacist (RPh) or Doctor of Pharmacy (PharmD).

We partner with your primary care provider or specialty care provider to oversee a range of health care needs, including:

We consult patients in our practice, which is located at EvergreenHealth Medical Center in Kirkland.

Patients who receive their care from EvergreenHealth Primary Care, Woodinville or EvergreenHealth Internal Medicine in Kirkland can see our on-site pharmacists. 


How to Get Started

To begin your care with a pharmacist, you will need a referral from your primary care physician.

Once we have received a referral from your physician, we will contact you to schedule your first appointment.

The visit with a pharmacist is most often covered by your insurance. A co-pay may apply.

What are the benefits to having a pharmacist available in a primary care practice?

Having a pharmacist fully integrated as a member of the primary care team means that patients will have the opportunity to benefit from the pharmacist’s experience and specialized medication knowledge in a convenient, familiar setting.

How will my Primary Care provider work with Pharmacist Provider Services?

To begin your care with a pharmacist, you will need a referral from your physician.

Pharmacists will not diagnose and won’t replace your primary care provider’s role in acute care.

Your primary care provider can assess whether this new care option may benefit you.


Do I have to be an EvergreenHealth Primary Care patient to see a pharmacist at an EvergreenHealth Primary Care location?

At this time, Pharmacist Provider Services are only available to patient of two practices: EvergreenHealth Primary Care – Woodinville and EvergreenHealth Internal Medicine + Primary Care in Kirkland. Patients will continue to need a referral from a doctor to make an appointment at any Pharmacist Provider Services location.


How will I be billed for my visit with the pharmacist?

EvergreenHealth Pharmacist Provider Services will bill for services received during each visit. This will be separate from any services received as part of your primary care visit.

You will receive a bill for each visit within 30-60 days of the date of service dependent on your individual insurers.

Similar to your other provider visits, your usual co-pay will apply to pharmacist visits.
