If you have already made your appointment, you can save time by using our online pre-registration form.

This pre-registration form is for the following services only:

• Maternity (use your expected due date)
• Surgery (Blue only)
• Diagnostic Imaging
• Comprehensive Procedures

All other services require in-person registration at the time of your appointment.

Please note:

• This is for first time patients only
• The online pre-admission form must be completed at least 72 business hours prior to your scheduled appointment.
• If you are within 72 hours, please contact our staff at (425) 899-2722 to complete your pre-registration.
