Important Patient Portal Update


Beginning April 28, 2020, EvergreenHealth Primary Care - Lakeshore Bothell and Totem Lake will be moving to the same Electronic Medical Record (EMR) used by EvergreenHealth, known as My Navigator.

This allows your providers across the continuum of care to have accurate, up-to-date and complete information about your health, at every point of care.

There are two things you can do to make the transition as seamless as possible:

Download your information from our current patient portal

The information on our current portal will not transfer to the new My Navigator portal, so if there is anything you want to save, please visit the portal and download the information before the portal closes on April 27. 

This could be lab results, screening results, immunization records, notes from your provider – any information you want to keep.


Set up your My Navigator patient portal account

Starting on April 28, we will begin using the new My Navigator patient portal, where you will be able to access your new health information as it accumulates.

If you already have an active My Navigator account with EvergreenHealth, there is no need to re-register.

If you do not have an active My Navigator account with EvergreenHealth, please register for one by following these instructions.
