Below are some questions you may have about OBGYN Care Tan.

Do I need to pay a copayment?

If your insurance requires a copay for an office visit, you will need to pay that amount at the time services are provided.

Do you accept credit cards?

Our office accepts VISA, Master Card and Discover credit cards, check or cash.

I don't have insurance; can I still see a doctor?

Contact our business office prior to making an appointment at 425.285.0093, so appropriate arrangements can be made for payment of services.

I had lab work done at EvergreenHealth Obstetrics & Gynecology Care, Tan but received a bill from another lab.  Who should I call if I have questions?

While your lab work was done in our office, it was sent to an outside laboratory. 

Please call the phone number shown on the bill from the laboratory, as we do not have access to laboratory billing information.

I won't have my new insurance card when I come in for my appointment.  What should I do?

If possible, check with your employer to find out:

  • Name of the insurance
  • Member ID number
  • Group number

If you are not able to obtain this information before the appointment, call our business office at 425.285.0093.

Without insurance information, the bill will be sent directly to you.

My health insurance has changed.  Do I need to do anything?

Each time you change health insurance companies, you will need to verify that EvergreenHealth Obstetrics & Gynecology Care, Tan is a participating provider with that insurance.

You will also need to bring you new insurance card with you to your next visit. 

When you check in for your appointment, notify the receptionist of the insurance change.  Your card will then be scanned into our computer system.

If you do not notify EvergreenHealth Obstetrics & Gynecology Care, Tan of your insurance change, charges will be forwarded to the previous plan and they will be denied. 

Because our office won't have the new insurance information, the statement will be sent directly to you. 

As soon as you give us your new insurance information we will be happy to re-submit the charges to the new insurance plan. 

My insurance informed me that I need to pay part or all of a bill, but I disagree with them.  What do I do?

If you are unsure or disagree with why your insurance company only paid a portion of the charges or has denied payment, please contact your insurance company. 

Although our business office may be able to provide some general information about why the insurance has not paid, we do not have information about your individual policy to answer your questions.

What happens if I switch insurance policies during pregnancy?

You will need to notify the business office at EvergreenHealth Obstetrics & Gynecology Care, Tan so your pregnancy care can be billed to the appropriate insurer by date of service.

Contact each insurer to receive information on your maternity benefits.

What if I can't pay my copayment at the time services are provided to me?

You will receive a statement from our office.

Why did I receive a statement that should have been billed to my insurance company?

If you received an explanation of benefits (EOB) from your insurance company, it should explain why services were not paid. 

If not, please contact our business office at 425.285.0093.

Why did I receive a statement for an amount I already paid?

Most likely the statement was sent out before your payment reached our office and was entered into your account.
