We value your time and want your appointments to go as smoothly as possible.

Below, you will find information and all of the items necessary to streamline the registration process and help us provide you with a great and timely experience.

Appointment Scheduling

Our schedulers are available weekdays 7:30am-5pm.

Please tell our receptionist the reason for your visit, so we can make sure to schedule the right amount of time for you.

If this is your first appointment:

Please complete these forms and bring a copy to your appointment.

To facilitate the transfer of your medical records from another practice, please complete the Request for Healthcare Information form and fax or mail it to your current provider.

Please allow at least two weeks for the request to be processed and your records to be received by EvergreenHealth Obstetrics & Gynecology Care, Tan.

If you are a returning patient:

You can request appointments using our patient portal.

Please complete these forms and bring a copy to your appointment.


Our Schedule

We try our best to stay on schedule, but due to the nature of our practice we are sometimes called away from the office.

We will make every effort to contact you in advance if a delay can be foreseen and arrange to reschedule your visit.

We appreciate your understanding, and apologize for any inconvenience this may cause.

Cancellation Notice:  we request 24-hours notice if you need to cancel your appointment. We will be happy to reschedule your appointment to a time that is more convenient for you.


What to Bring to Your Appointment

Your Insurance Card: we encourage you to contact your insurance company for information about your coverage, co-pays and benefits.

If your plan does not cover preventative care, or you do not have insurance, please contact our billing department to discuss payment options.

Co-pays: if applicable, co-pays are due at the time of service.


Contacting Your Physician

We ask that you to have your medication refills and clinical questions resolved during normal business hours. This policy will allow us to better meet our patients’ urgent medical needs after hours.

During Office Hours:

Call 425.285.0060 and choose option 3 to leave a detailed voicemail message.

Please include the spelling of your first and last name, date of birth and the phone number where you can be reached.

Your call for medical advice will be routed to a nurse who will consult with your physician and call you back.

This line is NOT for medical emergencies. If you are experiencing an emergency, please call 911.

After Hours: 

If you have a question or problem of an urgent nature when our office is closed, please call 425.285.0060.

The after-hours answering service will notify the physician on call, who will return your call as soon as possible.

The after-hours service is for urgent concerns; non-urgent issues and prescription refills should be addressed during regular business hours.

Obstetric Call System: 

During office hours, each physician tries to deliver her own patients if she is working in the office.

If your physician is not available, the physician on call will take care of you. Our physicians rotate responsibility at night, on weekends and on holidays.


Important Information about Your Visit

Billing
We will bill insurance as appropriate for the services you receive.

If your insurance requires a co-payment, this will be collected at the time of your appointment.

We accept most insurance plans as well as MasterCard, Visa, Discover and American Express.

We will also allow payment plans for services if discussed with our billing department in advance.

Our bookkeepers are available to assist you with any billing questions you may have.

Prescription Refills
For the renewal of a current prescription, please contact your pharmacy. If further information is needed for the prescription, the pharmacy will contact us.

Please plan ahead and allow 72 hours for the renewal of a prescription.

We do not refill prescriptions after normal business hours or on weekends.

By law, certain narcotic prescriptions cannot be phoned into a pharmacy.

Please let us know in advance and a written prescription, if appropriate, can be provided for you by your physician.

Onsite Lab
Our office has an onsite laboratory to draw blood and process urine tests; this enables our patients to complete their testing while still in our office.

We also have the capability to obtain certain results the same day if necessary.

Your physician or nurse will notify you with results by letter or by phone.

Disability Forms
Our office is happy to complete your disability form if medically indicated.

Please allow 7-to-10 business days for completion.

If more than one form is needed for the patient, or a family member of the patient, there is a processing fee of $20 per form.


Release of Information to another Clinic

If you are moving to a new clinic, this form will authorize us to release your Personal Health Information to your new provider.
