Prior to your appointment, please be sure you are enrolled in our patient portal My Navigator. At the time of scheduling, you should have been sent an email with instructions on how to sign up. If you did not receive anything, please follow this link to self-enroll. It is important that you enroll in the portal so that you can complete your online history prior to your appointment.”

If you are coming to EvergreenHealth Obstetrics & Gynecology Care, Coral from another clinic, these forms will instruct your former provider to release your Personal Health Information to us. Download and complete the form, then fax or mail to your former provider.

Our Schedule: your time is valuable, and we strive to run our schedule on time. That’s why we ask that you arrive 15 minutes before your appointment time to allow for the registration process. Out of respect for the patients that follow you in our office schedule, you may be asked to reschedule if you arrive late. When emergencies, deliveries and unforeseen delays take us out of the office, we will try to minimize your inconvenience by notifying you. If needed, appointments will be rescheduled. Siblings are welcome at prenatal visits, however, for your child’s safety and the consideration for other patients, we discourage unattended young children in our office waiting room.
Cancellation Notice: we require 48-hour notice of cancellations, and we reserve the right to charge for late cancellations and no-show appointments.
Lab Work: if your insurance plan is contracted with a particular lab, it is your responsibility to inform our clinical staff at the time of your appointment. If we are not informed and labs are sent out, we will be unable to do anything about the incurred lab fees.
National Coding Laws: your annual well-woman exam must be billed as preventive care. We cannot and will not re-bill visits with changed procedure or diagnosis codes as this constitutes insurance fraud and may jeopardize your provider’s medical license.
Identity Theft Law: the Federal Law now requires photo ID for your visit. We will take an in office photograph of all patients for our system to be compliant with the current identity theft law.
Prescription Renewals: for the renewal of a current prescription, please contact your pharmacy. If further information is needed for the prescription, the pharmacy will contact us. Please plan ahead and allow 72 hours for the renewal of a prescription.
Billing: we will bill insurance as appropriate for the services you receive. If your insurance requires a co-payment, this will be collected at the time of your appointment. If you have questions about billing, please feel free to contact our billing staff.
If you are moving to a new clinic, this form will authorize us to release your Personal Health Information to your new provider. Download and complete the form, then return to EvergreenHealth Obstetrics & Gynecology Care, Coral.
