Free parking is available in either the surface lot or the parking garage beneath DeYoung Pavilion.

Bike rack is available.

Finding the Office

Parking or Drop-off

  • Park in the DeYoung Pavilion Surface Parking or Underground Parking.
  • Drop passengers at the entrance of the Pavilion.

From Surface Parking

  • Enter the DeYoung Pavilion from the Surface Lot. Inside the Pavilion, you will arrive on the 1st floor. Take the elevator to the 5th floor.

From DeYoung Pavilion Underground Parking

  • Take the elevator directly to 5th floor.

On the 5th Floor

  • Proceed straight ahead to check-in.

Take Exit 20B for Totem Lake Blvd.
Continue straight onto 120th Ave NE.
Turn right onto NE 128th Street.
Follow the signs to the DeYoung Pavilion Entrance on your right.

Southbound on I-405
Take Exit 21 for NE 132nd Street.
At the first traffic circle, take the 4th exit onto NE 132nd Street.
At the next traffic circle, take the 1st exit onto 116th Ave NE/Totem Lake Blvd NE.
Turn left onto NE 128th Street.
Follow the signs to the DeYoung Pavilion Entrance on your right.

How to Prepare for Your First Appointment

  • Bring all related medical records and original diagnostic films (X-rays, MRI, etc.)
  • Bring a valid insurance card and driver's license.
  • Bring a list of your current medications.
  • Bring the referral from your primary care physician, if required.

Registration Options

  • Online registration; provide us with an email address, and we will send a link to your online registration two days prior to your appointment.
  • At the office; complete the forms on a tablet when you arrive at the office

At Your First Appointment

Your doctor will perform a thorough examination. We will provide a hospital gown to change into as needed. However, most patients are more comfortable in their own clothing, and we might suggest you bring in comfortable gym attire (i.e. shorts and t-shirt for men, shorts and sports bra for women).

You are the most important member of your rehabilitation team. Your physician, therapists, and other team members will guide your rehabilitation, but you must recognize that you have a responsibility to participate in, and take ownership of, any decisions involving your health care.

You will play an integral role in formulating your treatment plan, so ask questions about anything you do not understand.

At the EvergreenHealth Sport & Spine Care, our primary goals are to eliminate pain, maximize function, and restore your quality of life.

Because every patient is unique, we prescribe treatment based on a thorough evaluation and appropriate diagnostic testing. Furthermore, we feel that patients do best when they are properly educated regarding their diagnosis so they can have an active role in the decision-making and treatment process.

In our experience patients who participate in their own health care decisions are far more likely to maximize their recovery and restore function.
