In order to make your first visit go as smoothly as possible, please download and complete the child or adult new patient packet and bring to your visit.
We will be taking you photo for our records at your first visit. This enables us to accurately bill charges on your behalf and protect you against identity theft.

EvergreenHealth Primary Care, Lakeshore has a 24-hour on-call physician to answer questions and advise you about your care.
Our on-call physician has access to your medical record and can help determine the best course of action for your care.
If it can't wait, EvergreenHealth walk-in urgent care centers offer expanded hours in Kenmore, Mill Creek, Redmond, Sammamish and Woodinville.
When you are sick or in pain, you might think a visit to the ER is your best bet. Please keep in mind that a visit to the ER can result in longer wait times and higher costs compared to an office visit or an urgent care visit.
We understand that there are situations that require a trip to the ER. If a trip to the ER is absolutely necessary, please do not hesitate to seek care from your nearest Emergency Room. EvergreenHealth operates Emergency Rooms in Kirkland, Redmond and Monroe.
After your visit to the ER, please schedule a follow-up appointment with your Primary Care Physician as soon as possible.
When you need a prescription refilled, please call your pharmacy (local or mail order) and allow three business working days including Saturday to complete your request.
If you have no refills remaining, still contact the pharmacy and they will send a refill request directly to us.
If you are planning to go on vacation, please check your supply. If you need an early refill please notify your pharmacy.
For pick up information, please call the pharmacy and speak with a pharmacy staff person to see if your prescription is ready for pick up.
