Lakeshore Medicare/Medicaid Policy


EvergreenHealth Primary Care Lakeshore continuously evaluates Medicare options to develop policies that serve the best interests of our patients. After careful consideration, we have determined that we cannot accept NEW Medicare patients into our practice at this time.

All established patients (seen within the last three years) with Medicare can choose one of the following Medicare Advantage plans:

  • Aetna Med Advantage PPO and HMO 
  • Humana (Community HMO; Gold Plus HMO; Choice PPO)
  • Premera Medicare Advantage HMO
  • Providence Health Medicare Advantage HMO
  • Regence (MedAdvantage PPO and BlueAdvantage HMO)
  • Soundpath Health - HMO
  • United Healthcare (Dual Complete SNP; HMO; PPO; AARP MedicareComplete HMO)

Existing Lakeshore patients may also be seen if they have regular Medicare coverage or a combination of regular Medicare and a supplement.

All established patients (seen within the last three years) with Medicaid can choose either the Coordinated Care Plan of Washington Apple Health (managed care plan) or the traditional non-managed care option.
