If possible, please bring all of your medications, vitamins and supplements in their original container(s) to your first appointment.
Some medications, such as those for chronic pain or anxiety, may not be filled during your first visit in accordance with our controlled-substance policy.
Once you’ve established care with us, you’ll request refills directly from your pharmacy. To make sure you don’t run low on your medications, please plan for a 48- to 72-hour processing time.

If you need your provider to complete a form (return to work form, approval to join a gym, etc.), you can upload the form to us using the My Navigator patient portal.

In order to make sure you receive the health benefits to which you’re entitled, please present your ID and insurance card at every visit.
