Costs & Insurance Coverage

For a monthly fee, you get timely, round-the-clock access to a dedicated primary care doctor through EvergreenHealth Signature Care. You may be able to lower your family's long-term medical costs with preventive services from our personalized concierge care.

Pricing

Review the monthly Signature Care fee for:

  • Adults: $350/month
  • Couples (two adults living together): $560/month
  • Children from birth to age 17: $50/month with an adult membership
  • Young adults ages 18–26: $270/month with an adult membership

No Contract

You do not have to sign a long-term contract for Signature Care. Just give us 30 days' notice before ending your membership. If applicable, we'll prorate your fee at the time you end service.

Insurance Coverage

EvergreenHealth will continue to bill your insurance plan for medically necessary services that would typically be covered by them including provider exam, diagnostic testing, complete nutritional consultations, etc. Your payment to EvergreenHealth for participation in the Signature Care program is meant to cover the "enhanced" offerings of a concierge practice that wouldn't normally be covered by an insurance plan and thus isn't billed to them. You'll make any copays your insurance provider requires. View EvergreenHealth's accepted insurance plans.

Taxes & Health Savings Accounts

If you have a health savings account, ask your plan administrator if it will reimburse Signature Care expenses.

Ask your tax advisor if your monthly fee is tax deductible.
