At EvergreenHealth Pharmacist Provider Services for Diabetes, we practice guideline and evidence-based medicine, and we work in partnership with your primary care provider, endocrinologist, diabetes educator, and other healthcare professionals to help improve diabetes control.

Managing Diabetes with EvergreenHealth Pharmacist Provider Services

Diabetes is a long-term health condition where the body has trouble turning food into energy because it either doesn't make enough insulin or can't use it properly. Insulin is a hormone that helps move sugar (glucose) from your blood into your cells. When this process doesn't work well, blood sugar levels stay too high.

Over time, high blood sugar can damage many parts of the body. If diabetes is not well managed, it can increase the risk of serious health problems, including:

  • Heart disease and stroke
  • Kidney disease (which may lead to dialysis)
  • Nerve damage (which can cause pain, tingling, or numbness, especially in the feet)
  • Eye problems (including vision loss and blindness)
  • Foot problems (infections that can be hard to heal and may even lead to amputation)
  • Dental problems, like gum disease
  • Sexual and bladder problems, caused by nerve or blood vessel damage

Managing diabetes can help prevent or delay these complications. Keeping your blood sugar, blood pressure, and cholesterol levels in a healthy range, along with eating well, staying active, and taking medications when needed, can make a big difference in how you feel now and in the future.

What We Offer During Visits

A new patient visit is 60 minutes and follow up appointment is 30 minutes and includes:

  • A limited physical exam that can include measuring height, weight, waist circumference, heart rate and blood pressure.
  • A review of your medical history, medications, nutrition and physical activity.
  • A review of any laboratory test results.
  • A discussion of a medication plan to best help control glucose, which includes discussing how they work, common side effects, potential benefits to with cardiac and kidney health, and screening to see if discussed medications are appropriate for you.
  • Order and adjusting diabetes medications.
  • Order of other diabetes related medications to help ensure cardiac and kidney health in those with diabetes.
  • Order pertinent diabetes labs and diabetes glucose monitoring supplies.
  • A discussion of basic diet and exercise recommendations.
  • Help facilitate referrals such as Diabetes Education and Nutrition Services, sleep medicine, podiatry, nephrology, etc. (if needed).

How to Get Started

You will need a physician referral to enroll in diabetes management offered by EvergreenHealth Pharmacist Provider Services.

Your visit is typically covered by insurance, but we recommend that you verify this coverage with your individual health plan.

Once we have received a referral from your physician, we will contact you to schedule your first appointment.
