Home Health Services

Depending on your condition and need for skilled, short-term, intermittent home health services, your medical provider may refer you to receive one or more skilled services offered through EvergreenHealth Home Health.

Nursing Care assesses the effectiveness of medications and treatment for specific illnesses, including pain control, wound treatment, diabetic management, and catheter care. We also provide patient/family/caregiver education to maximize wellness and self-help.

Certified Wound Care Specialists (WOCN) are available for visits and consultation.

Psychiatric Nursing Service is available for those with mental health and behavioral issues.

Physical and Occupational Therapy can maximize independence through safety assessments and home modifications along with strengthening and home exercise programs to assist in recovery from physical limitations due to surgery or illness.

Psychiatric Occupational Therapy provides training to help family or other caregivers learn how to approach individuals with mental health, behavioral or dementia issues.

Speech Therapy and language rehabilitation assess and treat swallowing and speech difficulties as well as problems with memory and judgment.

Social Work offers assessment and counseling regarding financial and support needs, along with referrals to community resources and planning for long-term care.

Home Health Aides help with periodic bathing and personal hygiene.

All home health clinicians provide patient, family, and/or other caregiver training to improve patient safety, increase our patient's ability to successfully recover at home, provide support to those caring for the patient, and facilitate ageing at home.

Our Home Health clinicians work in collaboration with assisted or memory care facility staff and other paid caregivers who may help you with activities of daily living or companionship at home. Home health services are distinctly different and complementary to the private duty caregiving you may already receive.

When your home health treatments end, EvergreenHealth offers a seamless transition to additional services as needed. Those may include outpatient therapy, clinic-based services, outpatient palliative consultation or hospice services. Your home health team and community provider can help guide you as relevant.
