Our Home Health professionals work as a team with our patients and their physicians to optimize the patient's health at home. Our purpose is to help patients progress toward their goals, whether that means recovering from surgery, an illness or accident or managing a chronic disease.
Home Health services are intended to be short-term, intermittent, skilled services for people who are medically eligible, homebound and have been referred by their medical provider.
When to Contact Us
If you are receiving EvergreenHealth Home Health services, most of your visits will be pre-scheduled, but there may be situations that require consultation between visits or a call to your medical provider.
If you are experiencing an emergency, you should call 911.
If you are experiencing a sudden change in symptoms or experiencing severe pain, contact your community physician or provider.

- Experience a change in symptoms
- Need more information about your medications
Why Am I Being Discharged from Home Health?
Our goal is to collaborate with you to help you progress to your highest level of function and work with you to create the safest environment at your home. Because it is intended to be short-term, discharge planning is started shortly after you begin home health services. The therapists, nurses and other clinicians who are providing care to you in your home will talk with you about your goals for care and work with you to accomplish those goals.
There are several reasons that would lead to a home health discharge or transfer. Below we have outlined the criteria for discharge or transfer from our Home Health program.
- If your health and safety needs exceed what EvergreenHealth Home Health can provide for you.
- If you or your insurance will no longer pay for home health services, or if you no longer qualify for the Home Health benefit (i.e. you are no longer homebound).
- If your function has improved or stabilized to the point where the goals for home health care have been met.
If you have questions about your homebound status or if your insurance determines you are no longer eligible for services, your home health team will communicate timing with you and discharge you from services. Please contact your insurance carrier directly if you have questions about coverage.
