EvergreenHealth Home Health offers high-quality and compassionate nursing, physical, occupational and speech therapy, social work services and more for homebound individuals in King and Snohomish counties. Our trustworthy and experienced clinicians serve those recovering from injury or illness where they live, helping them reach their recovery goals. Patients receiving home health services are less likely to return to the hospital and experience fewer complications than those who do not.

Medicare, Medicaid, and most private insurance plans provide a specific home health care benefit, often with little to no out-of-pocket expense.

Linda Brabant, EvergreenHealth Executive Director of Home Health, shares how her team is committed to enhancing patients' quality of life by supporting their recovery journey in the comfort of their own homes all while reducing hospital stays.

Home Health care is short-term, intermittent skilled care available to help you recover from surgery, illness or an accident while staying in your home. Home health may also help you or your loved one learn to manage a chronic disease and understand medications. Home Health services are ordered by a medical provider and serve as an in-home extension of the medical care directed by your provider.

Your home health team will develop a personalized plan of care for you to work on recovery goals, independently or with your caregivers, as well as make recommendations for the next phase of recovery after home health services end.

How to Start Home Health Services

To qualify for Home Health, you must be considered homebound and in need of short-term, intermittent skilled care. Your medical provider may start a home health referral after a hospitalization, operation, skilled nursing stay or at a medical visit.

Individuals receive home health services for about 30-45 days. The frequency of visits during that time is decided in collaboration between the clinical home health team and your provider.

Some people transition to outpatient therapy at the end of their home health treatment plan, while others do not. Every patient and situation are unique; please discuss your specific circumstances and options with your care provider.
