To qualify for Home Health, you must be considered homebound and in need of short-term, intermittent skilled care. Your medical provider may start a home health referral after a hospitalization, operation, skilled nursing stay or at a medical visit.
Individuals receive home health services for about 30-45 days. The frequency of visits during that time is decided in collaboration between the clinical home health team and your provider.
Some people transition to outpatient therapy at the end of their home health treatment plan, while others do not. Every patient and situation are unique; please discuss your specific circumstances and options with your care provider. If you have further questions, please give our team a call at (425) 899-3300 to learn if EvergreenHealth Home Health is right for you.
