We value your time and want your appointments to go as smoothly as possible. Below, you will find information and all of the items necessary to streamline the registration process and help us provide you with a great and timely experience.

New Patients

We are temporarily closed to new patients.

Medical Records

If you see an EvergreenHealth Primary Care physician, we will have access to your medical records. If not, please ask your doctor to send us the notes from your last two office visits and any labs and brain/MRI reports. Our fax number is (425) 899-6818.

Scheduling Policy

Your time is valuable to us and our physicians are usually on time. Please call our office and let us know if you are running late so we can try to accommodate you along with our other patients.

Insurance

As a facility-based practice, we bill both a professional fee (for the medical team) and a facility fee (for the office visit) for most services rendered at the Geriatric Care office. This usually will result in higher patient out-of-pocket expense. If you are unsure how your insurance covers this facility and what you may need to pay, please contact your insurance company directly.

Accepted Insurance Plans

Medicare/Medicaid: we accept Medicare and Medicaid and most supplemental insurance plans. For your convenience, we will handle billing your insurance for you. Most patients with Medicare are only responsible for their usual 20 percent copay, and/or any unmet deductibles. If you have questions about services that Medicare does not cover, please ask one of our staff or providers.

Your First Visit

You should be accompanied by a family member or caregiver during the appointment. This will ensure that all information is noted in order to provide the safest level of care.

What to Expect At Your First Visit

Your first visit will be with a geriatrician and will last 60 minutes. The visit will most likely include further testing and evaluation by other members of the care team in addition to the time spent with the physician.

You will be provided with a care plan; any additional testing or treatments will be discussed with you.

A follow-up appointment may be scheduled in one-to-two months to see how the prescribed treatment is working. It is important that a family member or caregiver join you for this appointment.

Your primary care physician or provider will be sent a summary of the consultation evaluation with recommended testing, treatments and follow-up.

Our care team will be available to you and your family for any questions that you may have after the appointment.

Medications and Information

Please bring the following items to your first visit:

  • All medications that you are taking, including vitamins, herbal supplements and other non-prescriptive remedies
  • Insurance information
  • Eye glasses and hearing aids, if you have them
  • Advanced directives, if you have them
