Appointments

A referral from another physician is required even if your insurance does not require it.

All incoming referrals are screened to determine urgent and routine appointments.

Insurance

Before scheduling an appointment, we ask that you check with your insurance carrier to determine if EvergreenHealth Diabetes and Endocrinology Care is considered in-network or out-of-network.

If considered out-of-network, you must furnish a referral from your primary care provider for your office visit to be covered.
