EvergreenHealth Geriatric Care serves our community’s* older adults (65+) who have complex medical needs.
Ages 65-74 will need a referral from their primary care physician. No referral is needed for those 75 and older.
Just as you may be referred to a cardiologist for heart care, your primary care provider may refer you to us for help to improve your function, enhance your quality of life and maintain your independence.
Our experienced geriatric team will assess your medical conditions, cognitive status, functional ability and mental health needs to determine the right care for you.
Then we’ll work with your family and your other health care providers to put a care plan into place.
We also work with hospitalized older adults, and we’re able to reduce their hospital readmission rates by partnering with them, their families, their primary care physicians and other medical specialists to ensure a smooth transition from hospitalization to recovery.
Our team of specialists includes geriatricians, nurses, social workers and pharmacists - all with special training and experience assisting seniors to live their healthiest best.
Our practice serves senior adults (65+) in the service area of King County Public Hospital District #2 (Bothell, Duvall, Kenmore, Kirkland, Redmond, Sammamish and Woodinville).
If you’re age 65-74 and interested in seeking care from our practice, please talk with your primary care physician about a referral.
Ages 75 and older do not need a referral.
