We coordinate each patient's care needs with their primary care physician, specialty physicians, home care providers and other community resources.
We work with patients and their families to better understand topics such as living with chronic diseases, managing difficult behaviors and social problems associated with dementia, and making thoughtful end-of-life decisions.
This comprehensive evaluations and expert diagnosis by our team of physicians, pharmacists, social workers and nurses is then used to develop a treatment plan for patients and their families.
Neuropsychiatrist Dr. Reza Ghomi assists with patients who have cognitive decline where the underlying diagnosis is unknown, and screen for conditions such as frontal variant Alzheimer’s, lewy body dementia, frontotemporal dementia, primary progressive aphasia, posterior cortical atrophy and corticobasal degeneration, among others.
Dr. Ghomi will work to ensure your diagnosis is correct and discover which of your symptoms are part of the disease. Then you'll plan appropriately for the future to manage genetic implications, provide support and monitoring for both patients and caregivers and offer available research opportunities/treatments.
Appointments are available for current patients of the Geriatric Care clinic, with referral from your physician.
