Geriatric Care Memory Clinic

Are you concerned about your, or a loved one's, memory?

It's important to know that there’s a difference between normal memory changes related to aging, and memory loss associated with Alzheimer’s disease or other forms of dementia.

There are many causes of memory loss and an evaluation can help determine the cause, provide treatment, and support you and your family.

How can EvergreenHealth Geriatric Care help?

The team of professionals at Geriatric Care will look at all of the contributing factors to memory loss impacting seniors, determine a diagnosis and treatment plan, and provide support to help you and your family cope with memory changes.

What does a memory assessment include?

  • Your first appointment starts with memory testing by a social worker.  Then you will meet with a physician for a thorough history, physical examination, medication review, and discussion of labs and diagnostic imaging if indicated.
  • Your second appointment will be scheduled within 1-2 weeks to review the findings and to develop a plan of care that may include medications, exercise, future planning and support for you and your family.
  • While our clinic doesn't provide ongoing primary care, we will provide follow-up care and management of your memory issues. We work in partnership with your primary care physician or provider.

Insurance coverage

A comprehensive memory evaluation  is covered at 80% like any other outpatient Medicare service.  Most supplemental insurances cover the remaining cost, although policies vary so please check with yours to find out.
