Prior to your appointment, please be sure you are enrolled in our patient portal My Navigator. At the time of scheduling, you should have been sent an email with instructions on how to sign up. If you did not receive anything, please follow this link to self-enroll. It is important that you enroll in the portal so that you can complete your online history prior to your appointment.”
If you are coming to EvergreenHealth Midwifery Care from another clinic, this form will instruct your former provider to release your Personal Health Information to us. Download and complete the form, then fax or mail to your former provider.
Please download and complete the Returning Patients forms, and bring to your appointment:
By your 36th week of pregnancy, please complete and submit our online pre-registration. Use your due date as the expected service date.
If you are moving to a new clinic, this form will authorize us to release your Personal Health Information to your new provider.
