Please tell our receptionist the reason for your visit, so we can make sure to schedule the right amount of time for you.
We have two convenient ways to complete your new patient paperwork:
We'll have you verify your information and complete any forms due for renewal using our tablets in the office. Please arrive 15 minutes early to do this.
We try our best to stay on schedule, and will make every effort to contact you in advance if a delay can be foreseen and arrange to reschedule your visit.
We appreciate your understanding, and apologize for any inconvenience this may cause.
Cancellation Notice: we request 24-hours notice if you need to cancel your appointment. We will be happy to reschedule your appointment to a time that is more convenient for you.
Your Insurance Card: we encourage you to contact your insurance company for information about your coverage, co-pays and benefits.
If your plan does not cover preventative care, or you do not have insurance, please contact our billing department to discuss payment options.
Co-pays: if applicable, co-pays are due at the time of service.
We will bill insurance as appropriate for the services you receive.
If your insurance requires a co-payment, this will be collected at the time of your appointment.
We accept most insurance plans as well as MasterCard, Visa, Discover and American Express.
We will also allow payment plans for services if discussed with our billing department in advance.
Our bookkeepers are available to assist you with any billing questions you may have.
