Personal Information

I'm from Portland and have lived in Seattle since completing my fellowship. My wife and I have three kids and we love spending time with them and seeing them grow. We are an active family and enjoy living in the Northwest and taking advantage of all it has to offer.

Had forgotten to talk about the reason for my visit, since I had an additional condition that would need surgery that he wanted to schedule me for. Seemed knowledgeable and friendly, but that made me anxious.

all was good. forgot ask doc how he could feel/rate the varicocele on a contracted scrotum when it seems to be present or very large mostly when scrotum distended. wasn't sure what his Objective and Assessment measures gathered in the visit. forgot to ask.

Dr. didn't consult chart notes from first visit (I assume there were some). Caused me to have to call in again so I could get meds adjusted.
