As part of the vaccine distribution planning effort, the federal government and Washington State are considering many factors, such as prioritization of high-risk groups, vaccine supply and distribution, logistics, safety, and clinical guidance.
At EvergreenHealth, per Washington State Department of Health guidelines, we are currently vaccinating all Phase 1A (Tier 1 and Tier 2) and Phase 1B* eligible individuals.
*Reminder, these eligibility dates are estimated for each phase and are dependent on vaccine availability. EvergreenHealth will continue to follow the guidelines mandated by the federal government and Washington State to determine when each phase is eligible to receive the COVID-19 vaccine.

Yes, if you have insurance, we will bill your insurance for the administration of each vaccine.
Per the Washington State Department of Health, insurance plans are expected to cover the COVID-19 vaccine without any cost sharing. For more information, please view the Department of Health’s insurance coverage table (PDF).
At this time, there is no out-of-pocket cost for the COVID-19 vaccination and its administration.
The federal government will cover the cost of the COVID-19 vaccine. EvergreenHealth will charge a vaccine administration fee to give the vaccine. These fees will be billed to your insurance.
Yes, EvergreenHealth will administer the vaccine regardless of ability to pay. There will be no out-of-pocket cost to you.
Per the Washington State Department of Health, coverage of vaccine administration for uninsured patients will be provided through the federal Health Resources and Services Administration (HRSA) Provider Relief Fund.

Please be prepared to bring your identification card and insurance card (if you have insurance) to your vaccination appointment. If your appointment is for your second dose of the Moderna COVID-19 vaccine, please bring your CDC COVID-19 Vaccination Record Card with you to your appointment.
You should receive your second dose of the Moderna vaccine (EvergreenHealth is not administering Pfizer at this time) as close to 28 days from your first dose as possible. Ideally, this would be between day 24 and day 35 after your first dose. Those who schedule prior to day 24 will NOT be able to receive the second dose on day 23 or sooner.
