In addition to skill and training in all facets of general surgery, Dr. Hakkarainen also specializes in minimally invasive and remote-access surgery for the thyroid and parathyroid, including endoscopic transoral and robot-assisted approaches.
Dr. Hakkarainen is one of very few surgeons in the country trained in remote-access surgical techniques for endocrine surgery—particularly, the TransOral Endoscopic Thyroidectomy by Vestibular Approach (TOETVA) and the remote-access Bilateral Axilla-Breast Approach (BABA). Additionally, he is one of only three providers in Washington state with advanced training in endocrine (gland) surgery, with a focus on the thyroid, parathyroid and adrenal glands.

While traditional thyroid and parathyroid surgeries involve making an incision through the neck, advances in medical technology now allow for the same procedure to be performed using a remote-access, or hidden scar, approach. The TOETVA thyroidectomy technique allows for the removal of all, or a portion, of the thyroid gland via three small incisions in the mouth behind the lower lip. The BABA thyroidectomy technique uses four small incisions in the front and side of the chest. This method is not only used for thyroid surgery, but also for the removal of large glands or nodules—while still allowing scars to be hidden by everyday clothing. When performing the latest TOETVA and BABA surgical techniques, surgeons use state-of-the-art endoscopic equipment to remove thyroid or parathyroid glands through an incision in the lip or chest. These approaches not only enhance surgical precision, but also limit noticeable scarring for patients.
Dr. Hakkarainen’s specialized skill set is sought out nationally for training purposes, as well, given the limited number of physicians trained in these approaches. He recently returned to New York to help proctor a course for surgeons from across the United States seeking instruction and training on the TOETVA and BABA technique.
Dr. Hakkarainen believes his role as a surgeon is to help patients understand their conditions and partner with them to determine the best next steps in care. With the goal of providing a long-term solution to every individual he treats, Dr. Hakkarainen works closely with endocrinologists and primary care providers to identify the root cause of patients’ gland diseases to create personalized treatment plans best suited for their specific symptoms, while considering their personal values, goals of care and current life situations.
