Diabetes Prevention Program

The Diabetes Prevention Program is a structured year long program that includes dietary coaching, lifestyle interventions and moderate physical activity.

The goal is preventing the onset of diabetes in people who are pre-diabetic, or who had gestational diabetes during a pregnancy.

Primary goal is 5% weight loss.

16 intensive core sessions as a group, classroom-style setting providing practical training in:

  • Long-term dietary change
  • Increased physical activity
  • Behavior change strategies for weight control

After the 16 core sessions, less intensive monthly follow-up meetings help ensure that the participants maintain healthy behaviors.

The sessions will be facilitated by the Diabetes Education Staff at EvergreenHealth who have been certified as LifeStyle Coaches.

Who can participate?

The diabetes prevention program is recommended for people with:

  • Body Mass Index (height to weight ratio) of 25 or greater (23 or greater for Asian persons)
  • Hemoglobin A1c test result of 5.7 – 6.4%
    or
  • Fasting plasma glucose of 110-125 mg/dL within the last 12 months
    or
  • 2-hour plasma glucose of 140-199 mg/dL after the 75g oral glucose tolerance test
  • Positive screen for prediabetes based on the American Diabetes Association Type 2 Diabetes Risk Test (take the test online)
  • No previous diagnosis of diabetes or life-threatening conditions, mobility issues, etc. that would prohibit them from participating in the program

You do not need a referral from a provider to participate.

What is the time commitment?

The program runs for 1 year.

  • During the first three months of the program, you will meet about once a week.
  • During the second three months, you’ll meet every other week.
  • During the final six months, you'll meet monthly.

You may think you learned enough in the first six months and can skip the second half of the program. But you’d be cheating yourself!

Making lifestyle changes is an ongoing process.

Staying in the program for the full year is essential to help you stick to new habits and avoid slipping back into old habits.

And if you have not reached your goals in the first half of the program, your lifestyle coach and other group members can help you succeed.

When does the group meet?

The sessions will begin Jan. 9, 2017.  You'll receive details after you have registered.

The group meets weekly from 12-1 p.m. at EvergreenHealth Medical Center in Kirkland.
