Balance & Fall Prevention Program


Are you one of the 33 percent of adults over the age of 65 who will fall this year?

These falls can result in injury, hospitalization and ultimately a loss of your independence and quality of life.

EvergreenHealth's Fall Prevention Program is designed to identify fall risk factors and provide you with a plan to prevent the fall from happening.


Assessing Your Risk for a Fall

Our program is led by a team of therapists who specialize in fall prevention. Their goal is to help older adults maintain an active and independent lifestyle.

Your therapist will conduct an evidence-based evaluation that will review and assess:

  • Balance
  • Gait (walking)
  • Strength
  • Lower extremity sensation
  • Coordination
  • Home/community environmental fall risk factors
  • Vision impairments

After the assessment, your therapist will review the level of your fall risk.


Creating Your Fall Prevention Plan

You and your therapist will then create an individualized fall prevention plan, which may include one-to-one therapy or simply starting a home exercise program.

Treatment for fall prevention may include:

  • Balance exercise
  • Strength exercise
  • Cardio exercise
  • Patient education on home and community safety
  • Assistance with selection and use of appropriate assistive device (cane, walker, hiking poles)
  • Development of a home exercise program
  • Recommendations for appropriate community fall prevention/exercise programs

We believe that treating not just the individual, but addressing home and community safety, is the key to giving you back control and reducing your risk of falls.
