Safe, high-quality care is strengthened by the patients and families we serve. Their experiences help us understand what is working well, where we can improve and how we can better support people throughout their care journey.

The Patient and Family Advisory Council, known as PFAC, brings patients, family members and EvergreenHealth teams together in partnership. Members share feedback, offer ideas and help guide improvements that support a better care experience for patients and families across our system.

We are seeking patients and family members who reflect the diversity of our community and bring a range of experiences, backgrounds and perspectives.

Who Should Apply

You may be a good fit for PFAC if you:

  • Are an EvergreenHealth patient or family member of a patient who received care within the past three years
  • Are at least 18 years old
  • Live in Washington state
  • Want to help improve the care experience for patients and families
  • Feel comfortable sharing feedback based on your experience
  • Are able to attend at least four of six PFAC meetings each year
  • Are able to participate in meetings and occasional project work, about one to four hours per month
  • Are up to date on immunizations required for participation, or are willing to complete any required health screening steps

PFAC members meet in person and virtually to discuss ideas, share experiences and provide input. Members are asked to participate actively, respect confidentiality and work collaboratively with other PFAC members and EvergreenHealth teams.
