With your support, EvergreenHealth has championed community wellness through free behavioral health services for local students and health education classes.

These are just a few of the services EvergreenHealth provides thanks to you and the funding we receive from the community through our Public Hospital District levy. These programs are designed to address the diverse needs of the growing and changing community that EvergreenHealth serves.

Levy-Funded Program Directory

As a community-owned health system led by a community-elected board, EvergreenHealth is committed to ensuring the health and safety of you and everyone that calls our region home. Much of the amazing and important work that happens at EvergreenHealth wouldn't be possible without the community's support. This helps us lower the barriers to high-quality care for everyone.

Supporting the Community's Youth

Supporting Students in Crisis

EvergreenHealth provides social workers to Lake Washington School District high schools to assess students with high-risk behaviors and mental health concerns for free. The EvergreenHealth social workers meet with students 1:1, conduct groups, provide support to parents, and coordinate with school officials to keep students safe and connected to community providers. In 2024, our social workers helped 333 high school students with crisis intervention services.

High School Mental Health Therapists

Community funds also help the Northshore School District provide students access to mental health counselors. They meet individually and in small groups to address student needs. Supportive resources are also made available to students grieving the loss of a loved one. In the 2023-2024 school year, our therapists served 1,446 students and held 2,961 total visits.

Youth Mental Health

EvergreenHealth's levy-funded Youth Mental Health programs provide support and resources to help community members identify solutions and take action to promote awareness of the mental health challenges facing young adults.

Participants are taught how to recognize signs and symptoms of mental health challenges and crises, what to say and how to talk with someone, and how to refer to professional resources for help.

STEM Global Health Education Partnership

EvergreenHealth partners with our local school districts to give students the opportunity to learn about global health initiatives through job shadowing, attending Grand Rounds and hearing from guest speakers. These kinds of partnerships give students the opportunity to immerse themselves early on in learning about careers in science, health care and so many other industries.

Community Health Education Services

EvergreenHealth's Community Health Education Services provided a total of 875 health education classes in 2025, many of which are community-funded, enriching the lives of our community members with relevant health and wellness information. As a community-owned health system, our curriculum and programs look to address the diverse needs of our region while building a sense of community for you and your neighbors around common interests.

Wockner Hospice Care Center

The Wockner freestanding hospice center has 15 beds and serves hospice patients in King and Snohomish counties. It is the only hospice center in both counties and provides you or your loved ones with short-term acute care, with the goal of discharging home once symptoms have been stabilized.

Palliative Medicine

Our Palliative Medicine program uses levy funds to support a team that serves you and your family if any of your loved ones is facing a serious illness. These providers help patients and their families overcome physical and emotional challenges following a concerning diagnosis.

Patient Triage, Care Coordination and Access Program

The Patient Triage, Care Coordination and Access Program helps maintain the EvergreenHealth Medical Group's commitment to our community to provide safe, timely access to high quality, patient centered care. Levy funds help support an increase in timely access to care, improving patient care coordination and reducing unnecessary Emergency Department visits.

Additionally, levy funds support creating a framework for EvergreenHealth Medical Group's vision to ultimately consolidate all access and patient support services by the end of 2026, allowing care and coordination teams to expand their support into Specialty clinics.

Breaking Barriers to Health and Social Services

Community Healthcare Access Team (CHAT)

The Community Healthcare Access Team focuses on assessing and assisting community members in accessing healthcare resources, including long term care solutions, financial assistance, referrals to primary and specialty providers, and connecting members with insurance options. CHAT works in partnership with many community organizations to assist low-income and uninsured community residents who face barriers in accessing affordable health and social services.

Ambulatory Behavioral Health

Beginning in 2023, the Ambulatory Behavioral Health program provides therapy and psychiatric care, in collaboration with primary care providers, to established EvergreenHealth Primary Care patients. With the help of levy funding, the program plans to open availability to additional Primary Care clinics to better serve the needs of our community, eventually being able to offer immediate mental health support to patients across the EvergreenHealth service area.

The program is open to patients 13 years and older and greatly improves access to care by offering an in-house referral process for mental health services including therapy and psychiatry. Care providers are able to address chronic mental health issues, treating patients who have been unable to acquire successful referrals in our community.

Kirkland Fire Department Mobile Integrated Health

Kirkland Mobile Integrated Health (MIH) is an alternative EMS response and referral strategy that includes improving integration with behavioral health, linking low-acuity medical calls with an appropriate level of service, referring individuals to community-based services, and identifying root causes of need in complex care patients, among other activities. MIH delivers services directly to patients in their homes or communities, bridging gaps in traditional care models. Kirkland MIH is committed to equity and social justice with a program focus on those who are marginalized and face disproportionate health outcomes.

As a unit, MIH provides coordinated care for individuals with chronic conditions, limited access to care, non-emergent needs, substance use disorders, and mental health challenges. Kirkland MIH's primary objective, which aligns closely with many of EvergreenHealth's goals, is to improve patient outcomes, reduce unnecessary hospital visits and 9-1-1 calls, and enhance overall community health by providing timely, personalized care with connections to appropriate health and social services.

Health Services for Seniors

EvergreenHealth Geriatric Care

EvergreenHealth Geriatric Care is a dedicated team that develops specialized treatment plans for older adults with complex health needs. This includes expert assessment and treatment of dementia with behavioral health problems.

The transitional care management program provides expanded support and resources for patients as they transition from the hospital to home, or between clinic visits. The goal of this program is to reduce hospital readmission rates by addressing health issues as they arise.

The Chronic Care Management program is a vital part of the services provided by EvergreenHealth Geriatric Care. It is recognized by Medicare as a program that provides comprehensive and individualized care planning and management of chronic conditions.

Northshore Senior Center

Community funds allow us to provide health and wellness programs to older adults in the local community through the Northshore Senior Center (NSC) and the Adult Day Health Center. The funding includes seven evidence-based programs such as NSC's Enhance® Wellness program, designed to decrease the length of participants' hospital stays, alleviate symptoms of mood disorders and encourage older adults to maintain control of their lives through a participant-centered approach.

At the Northshore Senior Center in 2023, more than 1,500 seniors and their families participated in programs that helped improve their physical and cognitive functioning despite the challenges posed by the pandemic.

Additionally, the community-funded NSC Adult Day Health program offers virtual health and wellness classes to encourage movement, fine motor skills, strengthening and other mental and physical rehabilitation services.

Community funds also allowed us to serve 125 family caregivers in 2023 via programming, consultations and other services.

Kirkland Steppers

The Kirkland Stepper's Program was launched in the summer of 2003 with a diverse group of seniors—ranging from life-long walkers to individuals who had never exercised before. Now, 22 years later, the program continues to attract community members of all fitness levels, making it a beloved activity for seniors in the Kirkland area. One of the ongoing goals of the Peter Kirk Community Center is to combat the isolation that many older adults face as part of their daily lives. The Kirkland Stepper's is designed to be flexible, accommodating participants of all fitness levels, and provides a great opportunity for seniors to engage in regular physical activity in a welcoming community setting.

Throughout the twelve-week program, participants, or "Steppers," will walk to various local parks or meet up at designated locations. Walks generally last between 45 to 60 minutes, though participants have the option to extend their walk if they wish or shorten it if they prefer.

Each program or service that requests levy funding must annually demonstrate the community needs it addresses and detail program objectives, implementation plans and projected outcomes. Proposals are evaluated based on the need, and whether the overall benefit to the community justifies the resources.

EvergreenHealth's publicly elected Board of Commissioners has the final say on the distribution of levy funds each year with the recommendations of a standing levy committee.

While many of the levy-supported programs specifically address the special needs of under-served and high-risk community members, most of the programs are for use by the entire community.
