Leapfrog Hospital Safety Grade

The Leapfrog Group, a leading nonprofit watchdog organization on hospital quality and safety, has awarded EvergreenHealth an "A" rating for patient safety for Fall 2025. This is our fourteenth consecutive "A" rating, 2019-2025, a reflection of our ongoing commitment to protecting patients for preventable harm.

Leapfrog's Hospital Safety Grades assesses hospitals across the country on how well they protect patients from medical errors, injuries, accidents and infections. The grade is based on more than 30 national performance measures from the Centers for Medicare & Medicaid Services (CMS), the Leapfrog Hospital Survey and other data sources.

At Evergreen Health, patient safety is our priority. Earning this "A" grade again reinforces that our physicians, clinicians and staff deliver care in a safe environment built on continuous improvement, vigilance and teamwork.
